Check all impressions and models before packing to ensure the quality and accuracy of future crowns/partials.
For all remake/re-set cases, please return the original crown(s)/partials with model.
PLEASE NOTE: Our work is covered under a full 1 year warranty. There will be fees for remakes when:
A shade change is requested after completion
A preparation is re-prepped
If we question a case regarding the quality impression or model and the Dr. instructs us to completed it as it is.
All crown/bridge complete or try-in, removable denture/partial complete, valplast complete cases will require 9 working days, which does not include weekend or holidays.
We will give one week notice before any price changes (except in cases where there has been an extreme change in the international exchange rate or in the alloy market price).
A minimum of 50 units/day (1000 units/month) or more is required to receive special offers and discounts. For more information, please feel free to contact us.
Payment Terms: All accounts are payable within 25 days of statement date. Accounts not paid within the stated terms will be subject to COD status and a late charge of 2 percent of the unpaid balance.

A $30.00 charge will be assessed to your account on all returned checks.

50% of the shipping cost will be charged to your account for all returning packages using UPS NEXT DAY AIR/SAVER that contain less than three units
